10/24/2023 EnchantaVaughn Approved Behavior: QMHPA greeted Kaila upon arrival to the home setting. Upon arrival to the home setting, Kaila appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA informed Kaila that this session was going to focus on decreasing and cope with feelings of depression evident by stabilized mood, increased interactions and verbalizing positive feelings. Intervention QMHPA began the session by inquiring about Kaila’s current thoughts, feelings and emotions. QMHPA reviewed with Kaila appointments and her method of scheduling. QMHPA encouraged Kaila to notify her QMHPA of appointments to assist her with transportation. QMHPA discussed with Kaila about identifying triggers that causes depression as well as frustrations. QMHPA engaged Kaila in a mental health skills building activity. QMHPA explained to Kaila that the purpose of this activity was to explain to Kaila improving peer relationship by listening, create two options then reply instead of saying the first thing that comes to mind. QMHPA discussed with Kaila about learning and using coping skills when interacting with people throughout the community and using better judgement when meeting new people. QMHPA assisted Kaila with an exercise to help control her aggressive behaviors. QMHPA demonstrated with Kaila a group of words and Kaila has to say the first thing that comes to her head. QMHPA explained the same words to Kaila in a different order but allow some time, think then answer the best decision that comes to your mind. Response: Kaila appeared to be receptive to QMHPAs’ interventions and services that were provided to her during the session. Kaila displayed appropriate behaviors in a structured setting. Kaila appeared to be organized and keeps track of all of her appointments; however, she was frustrated about the transportation. Kaila discussed during the session that she do not like people who think they know it all or disrespectful. Kaila said she has always been the person who will say how she feels and keep it real. Kaila admitted to saying the first thing that comes to her mind without any thought. Kaila provided active participation in the exercise and displayed positive emotions. Kaila said she was able to recognize the difference in her thoughts when she thinks about more options. Kaila said when she is frustrated or angry she does not have time to think and that is what she is scared of most of the times because she does have a mental disability. Plan: QMHPA will utilize role play techniques addressing appropriate and inappropriate behaviors in social settings (1-2x weekly).
